(1) In case there are more Disputants or Opponents, their names, addresses,

(2) In disputes relating to monetary claims, the applicants should state the
precise amount claimed but where this cannot be exactly ascertained, the applicants
shall state the approximate amount claimed.
(3) When a society is a disputant a copy of the resolution of its committee or
Board of Directors shall accompany the application.
